  • Effect of laser lithotripsy of giant bile duct stones under endoscopic retrograde cholangiopancreatography and nursing management

    Abstract: Objective To compare the efficacy of dual-frequency dual-pulse laser lithotripsy under endoscopic retrograde cholangiopancreatography (ERCP) and traditional endoscopic mechanical lithotripsy in the treatment of patients with common bile duct stones, and summarize perioperative nursing measures. Methods From May 2018 to May 2022, the clinical data of 200 patients with common bile duct stones who underwent ERCP treatment and lithotripsy were retrospectively analyzed, of which 69 patients were treated with dual-frequency dual-pulse laser lithotripsy (Laser group) and 131 patients were treated with mechanical lithotripsy (Mechanical group), and the success rate of stone extraction, operation time, postoperative hospital stay time and complications of two groups of patients were compared. Results There was no significant difference in baseline data such as general conditions and preoperative clinical data between two groups (P>0.05). Neither neither Laser group nor Mechanical group had reported the perioperative death case. There were no significant difference in postoperative bleeding, postoperative pancreatitis and perforation complication rates in the two groups (P>0.05). The surgical time in the Laser group was significantly longer than that in the Mechanical group (P<0.05). The postoperative hospital stay was shorter in the Laser group than that in the Mechanical group(P<0.05), and the total complication rate and stone residue rate were lower in the Laser group than those in the Mechanical group (P<0.05). Conclusion The treatment of common bile duct stones by dual-frequency dual-pulse laser lithotripsy has better efficacy and lower complication rate than traditional surgical mechanical lithotripsy, but there is still room for improvement in the operation time.

  • Nursing study of the method of endoscopic jejunal nutrition tube insertion

    Abstract:

    Objective To investigate the safety and efficacy of jejunal nutrition tubes inserted into jejunum through therapeutic mirror accessory tubes for enteral nutrition. Methods A retrospective analysis was conducted on the clinical data of 90 patients with endoscopic placement of jejunal nutrition tubes in the Oriental Hospital of Beijing University of Traditional Chinese Medicine from January 2020 to January 2022, of which 60 patients were treated with jejunal nutrition tubes placed through oral biopsy hole forceps to jejunum (referred to as therapeutic lens group), and 30 patients were nasogascopically transnasal transnasal guide filament direct vision foreign body clamp clamping jejunal nutrition tube method (referred to as nasogastrometry group). Results All 90 patients were successfully placed in the upper section of the jejunum and successfully underwent enteral nutrition. The patient tolerated well during the catheterization process, and all patients were successfully placed at once. Nasogascopic group compared with therapeutic microscope group: the catheterization time is 15-60min compared with 5-30min, and the average 30min is 15min; The average depth of the tube was (100�10)cm ratio (120�10)cm; After catheterization, all patients have a good tolerance to enteral nutrition, the nutritional status of the patient has been significantly improved, and no extubation is required due to discomfort or inability to tolerate indwelling nutritional tubes; None of the complications such as perforation, gastrointestinal bleeding, and abdominal pain occurred. Conclusion The method of placing jejunal nutrition tube under the guidance of the guide wire of the transtherapy mirror accessory pipe has the advantages of short time and deeper catheter than the method of clamping the jejunal nutrition tube under the direct view of the nasogastric lens, and also has the characteristics of small trauma, easy operation, safety and high success rate, and is convenient for nursing, easy to accept patients, and is worthy of promotion and use by clinicians.

  • Quality nursing work in outpatient blood draw rooms for nursing studies

    Abstract:

    Objective: To analyze the effect of high-quality nursing on the improvement of outpatient blood collection quality and nursing quality. Methods: A total of 130 patients who received blood collection in the blood collection room from May 2020 to May 2022 were selected as the study objects, and the random number table method was used to divide them into observation group (65 cases) and control group (65 cases), and the quality nursing care and routine nursing were implemented respectively, and the application effects of the two nursing modes were compared. Results: The success rate of blood collection in the observation group (98.5%) was significantly higher than that of the control group (89.2%), the sample pass rate (96.9%) was significantly higher than that of the control group (87.7%), the incidence of nursing disputes (0) was significantly lower than that of the control group (6.2%), the degree of puncture pain was lower than that of the control group, and the satisfaction scores of various care care were higher than those of the control group <0.05)。 Conclusion: The application of high-quality nursing services in the outpatient blood collection room can help improve the success rate of blood collection and sample pass rate, and reduce nursing disputes, alleviate patient puncture pain, and improve their nursing satisfaction.

Results The baseline data such as general conditions and preoperative clinical data of the two groups were basically the same (P>0.05). There were no significant differences in postoperative bleeding, postoperative pancreatitis and perforation complication rates in the two groups (P>0.05). The surgical time in the laser group was significantly longer than that in the mechanical group (P<0.05), the postoperative hospital stay was shorter (P<0.05), and the total complication rate and stone residue rate were lower (P<0.05). Conclusion The treatment of common bile duct stones by dual-frequency dual-pulse laser lithotripsy has better efficacy and lower complication rate than traditional surgical mechanical lithotripsy, but there is still room for improvement in the operation time.

  • Clinical observation on effect of external application of Rutong powder in the treatment of acute mastitis at stagnation stage

    Abstract: ABSTRACT: Objective To investigate the effect of external application of Rutong powder in the treatment of stagnation stage mastitis. Methods? Totally 60 patients with stagnation stage mastitis were randomly divided into observation group and control group, with 30 cases in each group. Patients in both control group and observation group were treated with milk-ejection manipulation technique, and those in the observation group received the external application of Chinese herbal medicine Rutong powder. Results? The overall effective rate was 96.67%(29/30) in the observation group, which was higher than 83.33%(25/30) in the control group (P<0.05). Conclusion Milk-ejection manipulation technique combined with external application of Rutong powder is effective in relief of symptoms and discomfort in patients with acute mastitis at stagnation stage.
