Abstract: Objective To explore the adequacy of maintenance hemodialysis by using the Healthcare Failure Modes and Effects Analysis (HFMEA) method, so as to improve the dialysis quality and prognosis of hemodialysis patients. Methods A total of 78 patients who underwent maintenance hemodialysis in Zhongshan Hospital Fudan University (Xiamen Branch) from January 2021 to December 2021 were enrolled. The adequacy of hemodialysis was measured by using single-pool urea clearance (spKt/V). Patients were divided into the standard group (spKt/V≥1.2) and the non-standard group (spKt/V <1.2).HFMEA was used to measure and analyze the causes of hemodialysis insufficiency, and to formulate improvement measures. Results The healthcare failure mode and effect analysis (HFMEA) significantly improved dialysis adequacy in hemodialysis patients, the proportion of non-standard patients were reduced from 19.23% to 2.56% (P=0.002). Conclusion Management using the HFMEA approach could improve dialysis adequacy in hemodialysis patients, reduce dialysis complications, improve patients'' quality of life.

Abstract: Objective To evaluate the application of Plan-Do-Check-Action cycle(PDCA) model in management of breast cancer patients undergoing peripherally inserted central catheter (PICC)placement. Methods According to sequence of hospital admissions, totally 100 breast cancer patients undergoing PICC placement were divided into the control group and observation group, with 50 patients in each group. Routine PICC maintenance was carried out in the control group during the treatment, and PDCA cycle model was used in management of PICC placement in the observation group. The incidence of complications, treatment compliance and patient satisfaction were compared between two groups. Results The incidence rates of puncture-site infection, catheter blockage and fever induced by catheter-related infection in the observation group than those in the control group, and a significant difference was found in catheter blockage rate between two groups (P<0.05). Patients in the observation group had a better outcome of treatment compliance and satisfaction assessment than those in the control group (P<0.05). Conclusion The PDCA cycle mode in an effective tool in quality improvement of PICC maintenance for breast cancer patients. It is able to reduce the incidence of complications and improve the treatment compliance and satisfaction of patients.

Key issues of nursing included psychological care, preoperative hydration, guidance of dietary, observation of illness condition, prevention of complications, nutrition support and medication care. There are two severe cases of acute pancreatitis, one of which eventually died due to multiple organ failure, and the other was cured after drug treatment. In addition, two mild cases of acute pancreatitis, which were cured after drug treatment. As rare angiographic complication, acute pancreatitis is not easy to be detected early. The possibility of acute pancreatitis must be considered when a patient presents with abdominal pain, elevated creatinine, or multiple organ failure following an interventional procedure. Early detection and diagnosis, and timely treatment are beneficial to improve the cure rate of postoperative complications.
